Job Summary:

The Home Health Physical Therapist will be responsible for providing skilled physical therapy services to patients in their homes. This includes evaluating patients, developing and implementing individualized plans of care, and providing ongoing therapy services to promote optimal patient outcomes.

Responsibilities:
  • Evaluate patients to identify physical therapy needs and develop individualized plans of care.
  • Provide skilled physical therapy services to patients in their homes, including exercises, stretches, and other interventions as needed.
  • Monitor patient progress and adjust plans of care as necessary.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Document all patient interactions and therapy services provided in accordance with agency policies and regulatory requirements.
Requirements:
  • Current licensure as a Physical Therapist in the state of employment.
  • Minimum of one year of experience as a Physical Therapist in an acute setting.
  • Home care experience preferred.
  • Ability to effectively communicate and create positive impressions with patients, families, and healthcare teams.
  • Knowledgeable on therapy best practices and ability to make appropriate physical therapy judgments.
