Job Summary

This position provides effective advocacy for long-term care residents, ensuring their rights are not violated and their concerns are addressed. The successful candidate will provide comprehensive resident assessments, care, and discharge planning, while maintaining a high level of professionalism and integrity.

Key Responsibilities
  • Advocate for residents' rights and interests
  • Conduct comprehensive resident assessments and develop care plans
  • Collaborate with internal and external entities to ensure resident needs are met
  • Document resident needs, requests, and appointments in a timely and accurate manner
  • Review grievances and resident/family concerns and take appropriate steps towards resolution
  • Assess for trauma and develop care plans to meet residents' needs
  • Participate in pre-admission assessment and discharge planning processes
  • Facilitate communication between residents, families, and healthcare providers
Requirements
  • Bachelor's or Master's degree in Social Work
  • Current and active Social Work License from the State of New Mexico
  • Ability to read, write, speak, and understand the English language
  • Ability to deal tactfully with personnel, residents, family members, visitors, and the general public
  • Ability to function independently and work effectively with residents, personnel, and support agencies
Working Conditions

This position requires working in a fast-paced environment with frequent interruptions. The successful candidate must be able to maintain a high level of professionalism and integrity while working with residents, families, and healthcare providers. The position also requires the ability to work beyond normal working hours and on weekends and holidays when necessary.
