Job Summary

Ambercare is seeking a skilled Clinical Social Worker to join our hospice team in Santa Fe. As a key member of our interdisciplinary team, you will provide psychosocial assessments, counseling, and support to patients and their families.

Key Responsibilities:

  • Conduct thorough psychosocial assessments to identify patient needs and develop individualized plans of care.
  • Provide counseling and support to patients and their families to address stress, coping difficulties, and other psychosocial challenges.
  • Collaborate with healthcare professionals to ensure comprehensive care and coordination of services.
  • Assist patients and families with applications for benefits and resources, such as Supplemental Security Income and Medicaid.
  • Serve as a liaison between patients and families and community agencies to ensure access to necessary resources and services.

Requirements:

  • Master's degree in Social Work (MSW) from an accredited program.
  • Current licensure as a Licensed Clinical Social Worker (LCSW) in New Mexico.
  • Minimum of one year's experience in healthcare or hospice settings.
  • Valid driver's license and proof of insurance.
