Acute Care Medication Management Specialist

Novant Health is seeking a Clinical Pharmacist to join our acute care pharmacy team. This role delivers optimal medication management across all dimensions of care, ensuring patient safety and satisfaction.
The successful candidate will facilitate pharmaceutical care services, complete critical patient monitoring, and collaborate with healthcare providers to provide patient-centered care. A strong understanding of evidence-based best practices and regulatory standards is essential for this position.
Key Responsibilities:
- Facilitate pharmaceutical care services to ensure drug utilization activities align with patient care needs.
- Complete critical patient monitoring and reviews to identify, prevent, or mitigate drug-related problems.
- Collaborate with healthcare providers to provide patient-centered care.
Requirements:
A 4-year Bachelor's Degree in Pharmacy is required, with a Doctorate of Pharmacy preferred. Postgraduate year 1 residency or fellowship, or two years of experience as a registered pharmacist in health system pharmacy, are also required. Current Pharmacy licensure in the appropriate state(s) or completion within 30 days of start date is necessary. Board of Pharmacy Specialties Certification or other approved certification must be obtained and continuously maintained within 2 years of start date.
