Ambulatory Nursing Manager

CVS Health Careers is seeking a highly skilled and compassionate Nurse Practitioner to join our Primary Care team. As an Ambulatory Nursing Manager, you will play a vital role in providing high-quality, patient-centered care to our patients.
Key Responsibilities- Provide evidenced-based care to patients, working collaboratively with a dedicated clinical care team to drive better outcomes.
- Counsel and educate patients and families on health maintenance and respond to patient care inquiries.
- Evaluate and treat patients' medical needs with safe and high-quality treatment.
- Document all patient care within an electronic health record in accordance with current policies and procedures.
- Work as a member of the clinical care team providing care and coordination of our patients with internal and external colleagues ensuring the highest level of care is always provided for all patients.
- Provide patient-centered care that is compassionate, appropriate, and effective for the promotion of health, prevention of illness and treatment of disease.
- Refers to appropriate specialists and patient care components as appropriate.
- Collaborates with clinical care team to ensure the delivery of high quality, patient centric, longitudinal care.
- Participates in and in some cases leads clinical care team huddles.
- Demonstrates core values of CVS Health in all communications and interactions.
- Communicates effectively with members of the clinical care team, other health professionals and health-related agencies.
- On call rotation shared with providers in market for after hours patient clinical needs or shift coverage.
- Works effectively with clinical care team members.
- A minimum of 1 year of recent Primary Care experience as a Nurse Practitioner in either a family medicine or primary care practice preferred.
- Successful completion of CVS Minute Clinic primary care training program.
- Working knowledge and understanding of quality measurement in the management of chronic disease conditions.
- Ability to work remotely in a clinical care team culture.
- Ability to work without direct supervision and practice autonomously.
- Capacity to collaborate with professional colleagues frequently to ensure quality patient care.
