About the Role

We are seeking a skilled Clinical Care Manager to join our team at Brookdale Union. As a leader in clinical services, you will be responsible for managing the day-to-day operations of the community's healthcare department.

Key Responsibilities
  • Clinical Oversight: Ensure residents receive high-quality care and services that meet their unique needs and preferences.
  • Leadership and Coaching: Supervise and provide guidance to licensed nurses and direct care staff, fostering a collaborative environment that promotes excellence in care delivery.
  • Community Engagement: Build relationships with residents, families, physicians, and other healthcare providers to coordinate personalized care plans.
Requirements and Qualifications
  • Education and Experience: A state nursing license (LPN/LVN or RN) and a minimum of 3 years relevant experience, with clinical leadership experience preferred.
  • Tech Savvy: Strong working knowledge of technology and proficiency in Microsoft Office Suite and electronic documentation.
