About the Role:
This Clinical Care Navigator position plays a critical role in optimizing patients' recovery journeys. As a key member of our team, you will be responsible for completing weekly functional assessments, engaging the PAC inter-disciplinary care team, and facilitating informed decisions with patients and families. Key Responsibilities:
• Conduct thorough Skilled Nursing Facility (SNF) assessments using clinical skills and measurement tools
• Collaborate with healthcare teams to establish optimal discharge plans
• Engage with patients, families, or caregivers weekly and as needed
• Participate in SNF Rounds providing accurate information to senior managers or medical directors
• Review targets for Length of Stay (LOS), target outcomes, and discharge plans with providers and families

Requirements:
• Active, unrestricted registered clinical license in state of hire
• 5+ years of clinical experience working with adult or geriatric population
• Proficient with Microsoft Office applications
• Reliable mode of transportation and internet connection

Estimated Salary: $65,000 - $80,000 per year, based on location and experience
