Care Coordinator

We are seeking a dedicated Care Coordinator to join our team at Empowering People's Independence (EPI).
**Job Summary:**
As a Care Coordinator, you will play a vital role in supporting individuals with developmental disabilities, epilepsy, and brain injury in achieving their goals and improving their overall well-being.
**Key Responsibilities:**
Assist individuals in developing and implementing personalized plans to achieve their goals
Provide supervision, training, and assistance in daily living activities, including self-care, social skills, and community integration
Maintain accurate and up-to-date documentation and charting as required by regulations
Collaborate with management to ensure effective implementation of individual plans and program goals
Promote a safe and supportive work environment
**Requirements:**
High school diploma or equivalent; prior experience in the human services field is preferred
Valid New York State driver's license and reliable transportation
Ability to lift 50 pounds and perform physical demands of the job
Strong communication and interpersonal skills
**Work Environment:**
Varying shifts, including weekends and holidays
Exposure to changing weather conditions while traveling
Collaborative and dynamic work environment with a focus on supporting individuals with disabilities
