Job Summary

We are seeking a highly skilled and detail-oriented Patient Care Coordinator to join our team at Rochester Regional Health. As a key member of our healthcare team, you will be responsible for providing exceptional patient care and administrative support to our medical staff.

Key Responsibilities

• Utilize electronic medical records and office equipment to perform clerical tasks efficiently and effectively.
• Collaborate with physicians, nurse practitioners, and other healthcare professionals to ensure seamless patient care.
• Manage patient schedules, including regular office visits and adjustments as needed.
• Maintain accurate and up-to-date patient records, including documentation of patient encounters and communication with providers.
• Participate in patient education and support as requested by providers.
• Manage incoming and outgoing referrals, ensuring timely and accurate processing.
• Maintain a high level of communication with patients, providers, and staff to exceed expectations.

Requirements

• High school diploma or equivalent required.
• Medical terminology preferred.
• Ability to work in a fast-paced environment and prioritize tasks effectively.
• Excellent communication and interpersonal skills.
• Ability to maintain confidentiality and handle sensitive patient information.

Work Environment

This is a per diem position with variable scheduling. The ideal candidate will be able to work independently and as part of a team to provide exceptional patient care and administrative support.
