Key Responsibilities- Deliver and supervise patient care through comprehensive assessments, diagnosis, planning, implementation, and evaluation.
- Provide direct patient care to infants, children, and adolescents in their homes, adhering to physician orders and regulatory requirements.
- Administer prescribed medications, treatments, and therapies as ordered by the physician.
- Document timely, clear, and accurate clinical notes in accordance with the plan of care.
- Notify the RN Case Manager of any changes in the patient's condition.
- Provide education to patients and their families on specific details of diagnosis, treatment, and prevention.
- Collaborate with other professional disciplines to ensure patients receive quality care, necessary to achieve and exceed individual goals.
- Current State Registered Nursing License or compact license where applicable.
- One (1) Year Home Health Experience.
- One (1) Year Pediatric Experience.
- Current CPR Certification.
- Reliable Transportation.
The Registered Nurse - Home Health position requires regular physical demands, including bending, reaching, standing, stooping, sitting, twisting, talking, and hearing. The position also requires pushing and pulling 50 pounds, supporting 50 pounds of weight, and lifting 50 pounds, as well as assisting with weights of more than 100 pounds.
