About the Role

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at All Ways Caring HomeCare. As a Home Health Care Specialist, you will play a vital role in providing high-quality care and support to our clients in the comfort of their own homes.

Key Responsibilities
  • Provide personalized care to clients, adhering to their individualized plans of care and promoting their dignity and independence.
  • Monitor and report any abnormal vital signs or changes in client condition.
  • Administer medications, feedings, oxygen, and other treatments as needed, and document the results of these interventions.
  • Collaborate with other healthcare professionals, such as physical therapists, occupational therapists, and speech therapists, to ensure comprehensive care.
  • Implement and maintain infection control policies, reporting any possible communicable diseases or infections.
  • Accurately document all nursing actions, communication, and progress towards established goals.
  • Order and manage supplies, medications, and feedings to ensure adequate stock levels.
  • Ensure that all client needs are met in accordance with physician instructions.
  • Monitor and report any incidents involving clients, completing incident reports in a timely and accurate manner.
  • Maintain and report any faulty medical equipment to the designated maintenance team.
Requirements
  • Current license as an LPN or LVN in good standing from the state(s) of practice.
  • Current CPR certification.
  • Ability to work in a home-like setting.
  • Physical ability to bend, stoop, push, pull, sit, and walk for periods of time.
  • Able to utilize proper body mechanics while lifting 50 pounds.
  • Knowledge of individual service plans, including understanding of client rights and health needs.
  • Must have completed post-secondary coursework to facilitate licensure as an LPN or LVN.
