Key Responsibilities- Provide personalized care and support to clients, adhering to their individualized care plans and promoting their dignity and independence.
- Monitor and report any changes in clients' vital signs and health status, ensuring timely interventions and adjustments to care plans as needed.
- Administer medications, feedings, oxygen, and other treatments as prescribed, and document the outcomes and progress of care.
- Collaborate with other healthcare professionals, such as physical therapists, occupational therapists, and speech therapists, to ensure comprehensive care and support.
- Implement and maintain infection control policies and procedures, reporting any potential communicable diseases or infections to management.
- Accurately document all nursing actions, communication, and progress, ensuring that client goals and outcomes are met.
- Order and manage supplies, medications, and equipment as needed, ensuring adequate stock levels and timely replacements.
- Ensure that all client needs are met in accordance with physician instructions, and monitor incidents involving clients, reporting and documenting incidents as required.
- Maintain and report any issues with medical equipment, ensuring timely repairs and replacements.
- Current license as an LPN or LVN in good standing from the state(s) of practice.
- Current CPR certification.
- Ability to work in a home-like setting, with flexibility to adapt to changing client needs.
- Physical ability to bend, stoop, push, pull, sit, and walk for periods of time, with the capacity to lift 50 pounds using proper body mechanics.
- Knowledge of individualized service plans, including understanding of client rights and health needs.
- Completion of post-secondary coursework to facilitate licensure as an LPN or LVN.
