Nurse Practitioner or Physician Assistant

Landmark Health, a part of Optum, is seeking a Nurse Practitioner or Physician Assistant to join our team in delivering home-based medical care to patients. As a member of our interdisciplinary team, you will work collaboratively to manage complex patient panels and provide quality time to each patient, giving them the space, respect, compassion, and care they deserve.
Key Responsibilities:
• Perform initial comprehensive assessments on newly enrolled patients and provide ongoing care thereafter
• Develop patient management care plans and update them as needed when changes in condition warrant or following hospitalization
• Meet with patients and/or their legal representatives to review newly developed or modified care plans, involving the PCP or supervising physician when applicable
• Prescribe appropriate diagnostics and interventions to avoid unnecessary acute admissions
• Complete follow-up and post-discharge assessments according to documented standard operating procedure
Requirements:
• Completion of Nurse Practitioner or Physician Assistant accredited program
• Current state RN/NP or PA license
• Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or National Commission on Certification of Physician Assistants (NCCPA)
Preferred Qualifications:
• 2+ years of clinical experience, primary care, home-based medical care, geriatrics, or palliative care
• Knowledge of long-term care, community resources, and cost-effective alternatives regarding patient care delivery systems
• Knowledge of CPT, ICD-10, and HCPC codes
• Knowledge of clinical standards of care
