Job Description

The Primary Care Physician will be responsible for:

  • Providing comprehensive care to a panel of patients, including assessment, diagnosis, treatment, management, education, health promotion, and care coordination.
  • Leading their care team to ensure seamless patient care.
  • Working collaboratively with other healthcare professionals to improve health outcomes for seniors.
  • Developing and implementing effective treatment plans, taking into account each patient's unique needs and circumstances.
  • Maintaining accurate and up-to-date patient records, adhering to all relevant regulations and guidelines.
About You

To succeed in this role, you will need:

  • MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty.
  • Current State Medical License or ability to obtain one.
  • Board certification in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty.
  • Basic Life Support (BLS) certification from the American Heart Association (AHA) or American Red Cross (ARC).
  • Ability to maintain confidentiality and handle sensitive patient information.
  • Excellent communication and interpersonal skills.
  • Able to work independently and as part of a team.
