We are seeking a highly skilled and compassionate Registered Nurse II - Direct Care to join our team at the San Antonio State Hospital. As a key member of our healthcare team, you will provide direct care to individuals with intellectual and developmental disabilities, ensuring their physical and emotional well-being.
Responsibilities- Provide routine level nursing care, including assessment, treatment, and documentation of healthcare needs and issues.
- Evaluate and treat healthcare needs and issues, reporting to the appropriate person(s) as needed.
- Communicate effectively with direct care staff, physicians, administration, and other nursing staff to ensure optimal care as part of the interdisciplinary team.
- Notify the supervisor and PCP of trends and concerns to ensure consistent, high-quality care for all individuals served.
- Complete data entry, required documentation, and reports, maintaining accurate records.
- Assist with services provided by LVNs and Certified Medication Aides (CMA), as needed, and make recommendations for improvements.
- Participate in facility quality assurance/quality improvement (QA/QI) initiatives, as required.
- Respond to emergency situations to ensure safety and care of individuals and staff.
- Licensed to practice as a registered nurse in the State of Texas (or a state that recognizes reciprocity through the Nurse Licensure Compact).
- At least six (6) months of experience as a practicing registered nurse in a health agency, hospital, or healthcare facility.
- Bachelor's Degree is preferred.
