Job Summary:We are looking for a dedicated and experienced Registered Nurse to work as a Skilled Nursing Professional. In this role, you will be responsible for:
- Monitoring and assessing the delivery of home health field services
- Supervising the activities of Home Health Aides (HHAs) and providing case management support
- Developing and implementing care plans, and providing ongoing support and education to patients and caregivers
- Collaborating with primary care physicians and clinical teams to ensure comprehensive patient care
- Participating in quality assurance and education programs to improve patient outcomes
As a Skilled Nursing Professional, you will be responsible for:
- Conducting initial and ongoing supervisory field visits to patients' homes to assess their needs and provide support
- Providing training and education to HHAs on VNS Health guidelines and regulations
- Identifying continued training needs of HHAs and documenting information electronically
- Delivering start of care packets and providing an overall review with patients and caregivers
- Participating in special projects and performing other duties as assigned
To be successful in this role, you will need:
- A License and current registration to practice as a Registered Professional Nurse in the State of New York
- CPR/BLS Certification
- An Associate's Degree in Nursing from an approved program
- Minimum of two years in a clinical background as a Registered Nurse
- Home care experience in a licensed home care service agency or CHHA settings preferred
