We are seeking a highly skilled and compassionate Advanced Practice Provider to join our team at Wellbe Senior Medical. As a Geriatric Care Specialist, you will play a vital role in delivering high-quality, patient-centered care to our elderly patients in their homes.
Key Responsibilities- Model the Wellbe values and mission in care delivery for our patients.
- Follow state and federal regulatory agencies' rules and regulations.
- Provide primary care in the home-based setting for patients with chronic and acute illnesses and injuries.
- Complete initial patient screenings/physical examinations, create assessment/plan, and document in a timely manner.
- Order, interpret, and analyze diagnostic tests, determine the need for follow-up appointments and further treatment options.
- Prescribe and manage medications and treatments as appropriate.
- Update patient records and check records for accuracy at each patient appointment.
- Deliver the highest standard of quality care while maintaining company metrics.
- Focus clinical care delivery to support necessary items for documentation/coding, HEDIS measures, engagement of patients, and managing medical expenses.
- Master's Degree in Nurse Practitioner studies from an accredited, master's-level NP program.
- Active, unencumbered Registered Nurse and Nurse Practitioner licenses in the posted state and must reside in state.
- Current board certification, either AANC or AANP.
- BLS certified and an active NPI# as an NP.
- CPR certification.
- Active, unencumbered DEA licensure or eligible to apply for upon hire.
- Previous experience in home care, primary care, hospice, palliative care, or geriatrics strongly preferred.
- Ability to conduct in-home setting visits.
- Demonstrated role proficiency with Microsoft Office; Excel, Word, G-suite, etc.
- Proficient with office software, including Electronic Health Records, Scheduling Software, and Ring Central applications.
- Role model in courageous teamwork, integrity, ethics, and productivity.
- Experience in diagnosing and treatment of ongoing, chronic, and urgent medical/psychological conditions of older adults.
- Experience in panel management, primary care, geriatrics, internal medicine, palliative care.
- Experience in palliative care, and end-of-life communication.
- Excellent verbal, written, presentation, and interpersonal communication skills.
