Job Summary

We are seeking a skilled Post Acute Transition Specialist to join our team at CarePartners. This role will act as a coordinator between internal and external referral sources and our Post-Acute Service Lines, ensuring seamless transitions for patients.

Key Responsibilities
  • Establish relationships with hospitals, skilled nursing facilities, and other community referral sources to promote collaboration and education.
  • Conduct screenings and assessments to ensure appropriate patient placement through collaboration with SNF Discharge Planners & Inter-Disciplinary Teams, Ambulatory Care Managers, and other healthcare professionals.
  • Work closely with Post-Acute providers, SNFs, and physician practices to ensure a responsive and smooth flowing referral and admission process for our Post-Acute programs.
  • Collaborate with health system and community colleagues to promote quality-driven, best practice across the continuum.
Requirements

Required Education: Bachelor of Science in Nursing or related field.

Preferred Education: Master's in Nursing or related field.

Required License/Certifications: Current licensure as registered nurse with the North Carolina Board of Nursing or license in therapy discipline.

Preferred Experience: Minimum of two years recent experience in health-related field, with eight years preferred.
