**Geriatric Medicine Career Opportunity**
We are seeking a skilled Geriatric Medicine physician to join our team at Archway Physician Recruitment. As a Geriatric Medicine physician, you will play a vital role in providing high-quality care to seniors in Winston-Salem, NC.
**Job Summary:**

**Responsibilities:**
- Provide comprehensive care to patients with acute and chronic illnesses, including assessment, diagnosis, treatment, management, education, health promotion, and care coordination.
- Lead a care team consisting of a care promoter, care facilitator, and care coordinator to ensure seamless patient care.
- Engage with hospitalists and other healthcare professionals to ensure continuity of care for patients in the hospital or other settings.
- Participate in the management of your center and contribute to the recruitment of patients and additional providers as needed.
- Perform other duties as assigned and modified at the manager's discretion.
**Requirements:**
- Board certification or eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Experience in providing care to seniors with complex health needs.
- Strong communication and leadership skills.
- Ability to work collaboratively with a multidisciplinary team.
