Position Title: Patient Care Assistant

Overview:

The Patient Care Assistant plays a crucial role in delivering personal healthcare and support with daily living tasks, enhancing the quality of life for our clients while ensuring they can remain in their residences or facilities. This position requires strict adherence to confidentiality protocols and professional boundaries.

Key Responsibilities:

  • Administer, assist, assess, and document medical care, including:
    • Basic first aid procedures
    • Monitoring weight and vital signs
    • Providing catheter and bowel care, including intake and output tracking
    • Conducting range of motion exercises as directed by therapy professionals
    • Facilitating mobility, transfers, and positioning using wheelchairs and other assistive devices
    • Preparing meals tailored to dietary requirements
    • Addressing patients' specific conditions and needs within the scope of authorized tasks

Assist patients in maintaining personal hygiene, including:

  • Support with bathing, toileting, grooming, shaving, skin and nail care, oral hygiene, dressing, and eating

Documentation:

  • Complete and coordinate all necessary documentation, including:
    • Maintaining patient and family records upon visit completion
    • Reporting significant changes in patient conditions to the supervising RN

Qualifications:

  • High school diploma or GED
  • Current, unrestricted certification or proof of training as mandated by state regulations
  • Valid CPR certification
  • Proven ability in clinical assessments and adherence to care policies

Additional Requirements:

  • Valid driver's license and acceptable motor vehicle record
  • State-specific continuing education as required

Skills and Abilities:

  • Strong attention to detail
  • Effective time management
  • Problem-solving and conflict resolution skills
  • Excellent organizational and communication abilities

Physical Requirements:

  • Ability to communicate effectively in English
  • Capability to lift up to 50 pounds
  • Prolonged periods of walking, standing, bending, kneeling, reaching, and twisting
  • Ability to respond to emergency situations appropriately

Work Environment:

Must be adaptable to various environments, potentially encountering allergens and other conditions, including exposure to blood, bodily fluids, and infectious diseases.
