We are seeking a compassionate and dedicated Licensed Vocational Nurse to join our team at Westgate Gardens Care Center, a beautiful 140-bed facility. As a Licensed Vocational Nurse, you will play a vital role in providing high-quality patient care and contributing to the overall well-being of our residents.

Responsibilities:

  • Provide skilled nursing care to residents, including administering medications, wound care, and vital sign monitoring.
  • Develop and implement individualized care plans to meet the unique needs of each resident.
  • Collaborate with the interdisciplinary team to ensure seamless care transitions and effective communication.
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
  • Contribute to a positive and supportive work environment by promoting a culture of respect, empathy, and open communication.

Requirements:

  • Current LVN license in good standing.
  • At least 1 year of experience in skilled nursing, rehab, or post-acute care.
  • Ability to communicate effectively in writing and verbally in English.
  • Computer literacy and ability to navigate electronic medical records.
  • Strong critical thinking and problem-solving skills.
  • Ability to work independently and as part of a team.
