Job Summary

We are seeking a highly skilled Licensed Clinical Social Worker (LCSW) to join our team at Comprehensive Community Health Centers Inc. The ideal candidate will possess excellent assessment and brief intervention skills, with a solid knowledge of clinical social work practice, particularly in mental health and substance abuse and co-occurring disorders.

Key Responsibilities
  • Conduct accurate assessments of mental health issues in patients referred by medical providers
  • Provide direct clinical work with diverse patient populations of all ages
  • Develop person-centered treatment plans with patients using evidence-based screening tools
  • Assess patient motivation to engage in substance use and/or mental health treatment and engage or refer accordingly
  • Provide short-term clinical interventions to support patients in meeting their goals
  • Supervise MSW interns in providing mental health prevention services and case management
  • Document all assessments, interventions, and treatment plans according to departmental and payer guidelines
  • Contribute to a team approach promoting integration between Primary Care and Behavioral Health
Requirements
  • LCSW with relevant healthcare experience preferred
  • Two or more years of social work experience in a healthcare setting
  • Bilingual in English/Spanish required
