Job Summary

The Social Worker MSW PRN is responsible for providing non-clinical biopsychosocial assessments and interventions for inpatient floors and the outpatient emergency department. This role involves crisis intervention, patient/family interventions, high-risk screening, focused emotional support, and referrals for financial assistance or other identified resource needs.

Key Responsibilities
  • Collaborate with RN case managers, acute care nurses, physicians, insurance companies, and other staff members to identify and address patient needs.
  • Identify and address barriers to discharge and recovery, including financial, emotional, psychosocial, medical, and spiritual factors.
  • Perform thorough assessments and develop appropriate discharge plans for high-risk patients with discharge planning or psychosocial needs.
  • Participate in the interdisciplinary care planning process and perform proactive discharge planning.
  • Provide education to patients and caregivers regarding community resources and post-acute level of care criteria, benefits, and coverage.
Requirements

Master's Degree in Social Work is required. One year of experience or MSW internship is also required. Certification in Social Work or Licensed Clinical Social Worker (LCSW) is preferred.
