About the Role

VNS Health is seeking a skilled Psychiatric Social Worker to join our Mobile Crisis team. As a key member of our Behavioral Health team, you will provide vital client-centered care to individuals in need, across all stages of life and mental well-being.

Key Responsibilities
  • Deliver outreach services to mentally ill individuals in the community, providing in-home mental health intervention to those experiencing or at risk of psychosocial difficulties
  • Collaborate with another team member to perform psychosocial evaluations and assess mental health service needs of clients and their families through observation and interviewing
  • Create and execute short-term service plans for clients in collaboration with an interdisciplinary team
  • Respond promptly to dispatch calls, traveling from the office to the clients' homes within two hours
  • Ensure seamless program operations as a vital liaison to community agencies and stepping in for the Program Coordinator as needed
  • Ensure uninterrupted comprehensive care from counseling and care management to long-term supplemental treatment referrals and follow-up
  • Prepare case histories and prepares and maintains case records, in accordance with the Program record-keeping mechanism
  • Encourage resistant clients to accept mental health services through interventions with clients and/or family members and friends concerned with the client's welfare
  • May provide clinical supervision for Mental Health Technician, graduate Social Worker students or junior staff Social Workers
Requirements
  • Current registration to practice as a Licensed Master Social Worker in New York State Required as determined by operational/regional needs
  • Valid driver's license may be required
  • Minimum of one year experience as a Social Worker in a health care setting required as determined by operational needs, bilingual skills may be required Must be able to walk up and down steps to access patient residences
