Job Overview

POSITION TITLE: Youth and Family Care Coordinator

SERVICE AREA: Care Coordination Services

PROGRAM: Care Coordination

EMPLOYMENT TYPE: Full Time (35 hours per week), Hybrid Position

CLASSIFICATION: Hourly

COMPENSATION: $22.37-$23.43 per hour

SUPERVISOR: Supervisor-Care Coordination Services

EDUCATION: Bachelor's Degree in Social Work or a related discipline.

PROFESSIONAL EXPERIENCE: A minimum of 2 years of relevant experience with Serious Emotional Disturbance (SED) or Serious and Persistent Mental Illness (SPMI) or chronic physical health conditions; experience in linking and coordinating community and emergency services; familiarity with substance abuse treatment is preferred.

**Travel is required throughout the designated service areas.**

AGENCY REQUIREMENTS:

AGENCY EXPECTATIONS:

  • Demonstrate understanding of the program, mission, and services of Pinnacle Community Services.
  • Participate in agency staff training and meetings.
  • Advocate for cultural diversity and sensitivity.
  • Report any suspected incidents of child abuse or neglect to the appropriate authorities.

GENERAL RESPONSIBILITIES:

  • Provide direct social work services to youth aged 5-18 and young adults aged 18-25.
  • Maintain ongoing communication with youth, families, community providers, and educational institutions while meeting service expectations.
  • Coordinate with service providers and conduct biannual team meetings to develop personalized care plans.
  • Ensure youth have access to on-call services for after-hours assistance.
  • Provide on-call services as scheduled.
  • Prepare accurate and timely case documentation.
  • Meet regularly with the Program Supervisor.
  • Engage in mandatory and optional training sessions.
  • Advocate for cultural diversity as appropriate.
  • Perform additional duties as assigned by administration.

ADDITIONAL RESPONSIBILITIES:

  • Conduct assessments to determine the level of care and needs for youth.
  • Maintain accurate records for state-level tracking.
  • Verify Medicaid status regularly.
  • Provide support and resources for employment, education, and housing to promote youth health and development.
  • Engage young adults in their goal-setting processes.
