Job Summary

As a RN Case Manager at SSM Health, you will play a critical role in ensuring patients receive the best possible care and support during their hospital stay and beyond. Your primary responsibility will be to assess, develop, and implement discharge plans that meet the unique needs of each patient, working closely with the interdisciplinary team to ensure a smooth transition to the next level of care.

Key Responsibilities:

  • Conduct thorough assessments of patients to identify their discharge planning needs and develop individualized plans to meet those needs.
  • Collaborate with the healthcare team to ensure that patients receive the necessary care and support to achieve their discharge goals.
  • Identify and escalate any barriers or gaps in care that may impact a patient's discharge plan.
  • Develop and implement strategies to improve patient outcomes and reduce readmissions.

Requirements:

  • Graduate of an accredited school of nursing.
  • Three years of acute care experience.
  • Registered Professional Nurse (RN) licensure in the state of Illinois, Missouri, Oklahoma, or Wisconsin.
