Job Summary

As a RN Case Manager at SSM Health, you will play a vital role in ensuring patients receive seamless care transitions from hospital to home. This position requires a skilled and compassionate nurse who can assess patients' needs, develop personalized discharge plans, and collaborate with interdisciplinary teams to achieve optimal outcomes.

Key Responsibilities
  • Conduct thorough assessments of patients' medical, social, and environmental needs to identify potential barriers to discharge.
  • Develop and implement individualized discharge plans that address patients' unique needs and circumstances.
  • Collaborate with healthcare teams, including physicians, nurses, and social workers, to ensure timely and effective care transitions.
  • Identify and escalate potential readmission risks, developing strategies to mitigate these risks and promote patient safety.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.
Requirements
  • Graduate of an accredited nursing program.
  • Current RN licensure in the state of Illinois, Missouri, Oklahoma, or Wisconsin.
  • Three years of acute care experience, preferably in case management or a related field.
