Job Summary

At SSM Health, we are seeking a dedicated and compassionate RN Case Manager to join our team. As a key member of our healthcare team, you will play a vital role in ensuring the seamless transition of patients from hospital to home, while promoting optimal health outcomes and patient satisfaction.

Key Responsibilities
  • Assess patients' discharge planning needs and develop personalized plans to ensure a safe and effective transition to the next level of care.
  • Collaborate with interdisciplinary teams to identify and address potential barriers to discharge, including social determinants of health and access to community resources.
  • Utilize evidence-based practices and clinical expertise to identify high-risk patients and develop targeted interventions to prevent readmissions.
  • Participate in quality improvement initiatives to enhance patient care and outcomes, while promoting a culture of safety and transparency.
Requirements
  • Graduate of an accredited school of nursing with a current RN license in the state of Illinois, Missouri, Oklahoma, or Wisconsin.
  • Three years of acute care experience, with a focus on case management or a related field.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
