Job Title: Clinical Care Assistant

We are seeking a compassionate and skilled Clinical Care Assistant to join our team at The Suites at Someren Glen. As a Clinical Care Assistant, you will play a vital role in providing high-quality care to our residents, ensuring their physical, emotional, and social needs are met.

Job Summary

The Clinical Care Assistant will work on a flexible, as-needed basis, providing clinical services to all residents residing at The Suites at Someren Glen. This will include skilled nursing, assisted living, and all shifts. The successful candidate will coordinate care and services based on resident needs and individual preferences, as determined by ongoing assessment and evaluation.

Key Responsibilities
  • Provide daily direction and/or assistance with residents' activities of daily living.
  • Become familiar with residents' preferences to personalize their care.
  • Assist residents with social, personal, and health care needs as required.
  • Responsible for knowing and properly applying safety, infection control, and risk management procedures.
  • Make decisions in an emergency and notify appropriate personnel.
  • Communicate regularly with relevant nursing supervisors regarding residents' status and condition.
  • Accurately document and promptly report discrepancies to appropriate personnel.
Requirements
  • High School Diploma or equivalent.
  • Certified Nursing Assistant License-active and in good standing.
  • Ability to work well with different teams in different communities.
  • 3-6 months experience working with senior adults preferred.
  • Excellent customer service skills.
Working Conditions
  • Position requires strenuous physical work. Heavy lifting, pushing, or pulling objects up to 100 pounds occasionally and/or up to 50 pounds frequently.
  • Exposure to blood borne pathogens, body excretions, and communicable diseases possible.
  • Sits, stands, bends, and moves frequently during working hours.
  • Is subject to frequent interruptions.
  • Interacts with residents, family members, visitors frequently.
