Key Responsibilities:
- Provide case management services, crisis intervention, and skills training to clients and their families.
- Conduct individual, family, and group counseling sessions to support clients in achieving their rehabilitation goals.
- Develop and implement individualized treatment plans, ensuring continuity of care and adherence to regulatory standards.
- Collaborate with multidisciplinary teams to facilitate client recovery and community integration.
- Document client progress and maintain accurate records, adhering to established timelines and protocols.
Requirements:
- Bachelor's degree in a relevant field, with a minimum of two years' experience in mental health services.
- Demonstrated education, training, or experience in mental health services, with a focus on rehabilitation and community support.
- MHP designation and valid driver's license required.
- Excellent written and verbal communication skills, with the ability to adapt communication styles to meet client needs.
