Job Title: Quality Improvement Program Manager

At Santa Clara Family Health Plan, we are seeking a highly skilled Quality Improvement Program Manager to join our team. This is a critical role that will play a key part in driving our organization's quality improvement initiatives and ensuring we meet the highest standards of care for our members.

Job Summary:

The Quality Improvement Program Manager will be responsible for developing and executing a comprehensive strategy to improve the quality performance of our organization. This will involve working closely with internal teams, external vendors, and healthcare providers to identify areas for improvement and implement effective solutions.

Key Responsibilities:
  1. Develop and implement quality improvement initiatives to improve performance on HEDIS and MCAS measures.
  2. Collaborate with internal teams and external stakeholders to identify areas for improvement and develop effective solutions.
  3. Monitor and analyze data to identify trends and opportunities for improvement.
  4. Present findings and recommendations to management and other stakeholders.
  5. Develop and maintain relationships with healthcare providers and vendors to ensure effective collaboration and communication.
  6. Stay up-to-date with regulatory requirements and changes to ensure compliance.
Requirements:
  1. Bachelor's degree in Public Health, Healthcare, or a related field.
  2. Minimum three years of experience in quality improvement in a healthcare setting.
  3. Knowledge of Medicaid principles and practices.
  4. Proven ability to design and execute strategic plans for continuous improvement initiatives.
  5. HEDIS experience.
  6. Medicaid plan or ACOs experience.
Working Conditions:

This is a full-time position that will be based in our office. The successful candidate will be required to work in a fast-paced environment and be able to prioritize multiple tasks and deadlines.

Physical Requirements:

The successful candidate will be required to perform the essential functions of this job, with or without reasonable accommodation. This includes regular bending, reaching, and sitting for extended periods of time.
