Job Title: Integrated Community Support Professional

At Credent Care LLC, we are seeking a dedicated and compassionate Integrated Community Support Professional to join our team. As a key member of our home and community-based service provider, you will play a vital role in promoting independence, community integration, and personal growth of program participants.

Responsibilities:
  • Foster a safe and supportive environment that encourages independence, community integration, and individual personal growth.
  • Enhance community engagement and individual socialization through various activities and outings.
  • Assist with transportation needs and preferences, ensuring safe and reliable transportation for program participants.
  • Provide safety education and training on pedestrian practices, personal safety, and emergency preparedness.
  • Support the development of sensory and motor skills, adapting tasks to meet individual abilities and interests.
  • Collaborate with program participants to identify and access healthcare, education, and employment services, as well as research available resources and guidance on eligibility criteria.
  • Facilitate introductions and socialization skills training, promoting positive relationships with supportive peers, family, and community members.
  • Provide training on effective communication strategies, active listening, assertiveness, and conflict resolution, as well as role-playing exercises and real-life applications.
  • Assist with daily living tasks, such as cleaning, organizing, and meal planning, promoting independence and self-sufficiency.
  • Support the development of individualized strategies and care plans, regularly monitoring progress toward goals and making adjustments as needed.
  • Collaborate with the client's support network for consistency and ensure that all team members are aware of the individual's needs and goals.
Qualifications:
  • High school diploma or equivalent (some positions may require additional education or certifications)
  • Previous experience in a similar role or working with individuals with developmental disabilities, mental health concerns, or related populations is preferred.
  • Demonstrated ability to provide compassionate, person-centered care and support.
  • Strong communication and interpersonal skills with the ability to work effectively within a team.
  • Ability to maintain professional boundaries, confidentiality, and ethical standards.
  • Basic computer skills and proficiency in documenting services accurately.
  • Physical ability to assist individuals with mobility, lifting, and personal care needs as required.
  • Must have a valid driver's license with an acceptable driving record.
  • Current auto liability insurance.
  • Ability to drive own vehicle while working.
  • Acceptable clearance of background check.
  • Must be dependable, adaptable, and flexible.
  • Must be observant and detail-oriented.
  • Positive role model for others and able to work on a team.
  • Committed to creating a respectful and collaborative environment.
  • Person-centered, patient, and kind.
  • Must have good decision-making, communication, and time-management skills.
  • Must be responsible, reliable, and able to carry out job functions without direct supervision.
  • 18 years or older.
