Job Summary:

We are looking for a dedicated and experienced Physical Therapist to work with our pediatric patients in a home health setting. As a Pediatric Physical Therapist, you will be responsible for assessing, planning, and implementing individualized physical therapy programs to help our patients achieve their goals and improve their quality of life.

Responsibilities:

  • Conduct thorough assessments of patients to identify their physical therapy needs and develop individualized treatment plans
  • Implement and modify treatment plans as needed to ensure optimal patient outcomes
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination
  • Collaborate with other healthcare professionals to ensure comprehensive care and address any patient concerns
  • Document patient progress and outcomes in a timely and accurate manner

Requirements:

  • Current licensure as a Physical Therapist in the state of practice
  • Experience working with pediatric patients in a home health setting
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Valid driver's license and reliable transportation
