As a Program Manager at Evergreen Supported Living, you will oversee a dedicated team and manage a caseload of individuals with developmental disabilities. Your role will encompass the administration of client programs, household management, and the professional development of your team members. You will work collaboratively to create and implement Individualized Instruction and Support Plans (IISP) and Functional Assessments/Positive Behavior Support Plans (FA/PBSPs), ensuring that all daily objectives and compliance with established policies are met.
Key Responsibilities:
- Provide direct care support for clients as necessary to ensure compliance with contractual obligations.
- Supervise and mentor Direct Support Professionals and Assistant Program Managers.
- Participate in an on-call rotation to maintain quality care and operational efficiency.
- Manage resources effectively related to client caseloads in assigned programs.
- Collaborate with the team to develop, implement, and review client IISP and FA/PBSPs, ensuring thorough audits and updates.
- Work alongside the Client Financial Program Manager to complete annual Individual Financial Plans (IFP) and oversee client financial activities.
- Partner with the Quality Management and Training Program Manager to ensure high-quality care and successful client outcomes through accurate documentation and adherence to policies.
- Maintain organized and secure client files at both the administrative office and site locations.
- Oversee Medication Administration Records (MARs) for accuracy and identify training opportunities.
- Support Assistant Program Managers in medication management and compliance with agency policies.
- Monitor and adapt program activities to enhance clients' quality of life.
- Train and coach staff to ensure documentation is complete and accurate.
- Ensure completion of safety meetings and fire drills, including inventory of first aid supplies.
- Manage site cleanliness and maintenance, ensuring agency vehicles are safe and ready for use.
- Coordinate client outings and maintain the monthly activities calendar.
- Ensure all client appointments are scheduled and communicated effectively with the care team.
- Oversee household supplies and collaborate with staff to prepare weekly grocery lists.
- Facilitate training and support for clients in personal care and home management.
- Implement Positive Behavioral Support strategies and report on their effectiveness.
- Engage with families, guardians, and other stakeholders in a professional manner.
- Provide transportation support for clients as needed.
- Utilize individualized plans to assist clients in achieving their goals.
- Promote teamwork and respectful interactions among staff.
- Maintain daily progress notes and other documentation in accordance with policies.
- Lead monthly staff meetings and training sessions.
- Conduct performance reviews with Assistant Program Managers and Direct Support Professionals.
- Encourage client involvement in daily activities to foster independence.
- Ensure compliance with training and documentation requirements.
- Assist clients in managing their health and participating in healthcare appointments.
- Administer medications and perform delegated nursing tasks under supervision.
- Support clients with personal hygiene tasks in a respectful manner.
- Maintain cleanliness in client homes and agency spaces according to infection control procedures.
- Promote fiscal responsibility and compliance with budgetary guidelines.
- Embody the values outlined in the DSHS Residential Service Guidelines in all aspects of client care.
- Collaborate with the management team to ensure effective agency operations.
- Other responsibilities as assigned.
- Minimum age of 18 years.
- Bachelor's degree or equivalent experience in working with individuals with developmental or intellectual disabilities.
- Completion of Evergreen's Management Training Program.
- At least one year of experience as an Assistant Program Manager.
- Certifications in CPR, First Aid, Bloodborne Pathogens, and Food Handling.
- Nursing Assistant Registration (NAR) is preferred.
- Strong communication skills, both written and verbal.
- Knowledge of relevant WACs, DDA Policies, and applicable laws.
- Ability to effectively teach and mentor others.
- Valid state driver's license with a clean driving record.
- Successful completion of a DSHS background check.
- Physical capability to assist clients as needed.
- Flexibility to work non-traditional hours, including evenings and weekends.
- Familiarity with medical terminology is advantageous.
- Proven experience in managing staff and meeting deadlines.
- Full vaccination for Covid-19 is required.
