Position Summary:Under the supervision of the Team Leader, this role involves delivering case management and support coordination services in various settings, including home, office, and community environments. The focus is on children and adolescents facing serious emotional disturbances (SED), developmental disabilities, and/or co-occurring disorders. Responsibilities include:
- Collaborating with the child, family, and natural support systems to develop a personalized care plan.
- Conducting assessments, linking families to necessary services, and monitoring their progress.
- Coordinating with educational institutions, legal systems, child welfare, and other community resources.
- Working alongside families, professionals, and support networks to foster a supportive environment for the child.
- Performing related duties as necessary.
Qualifications:
Educational Background:
- Bachelor's degree in Social Work or a related human services field (such as Education, Counseling/Guidance, or Psychology).
Experience:
- At least one (1) year of experience in evaluating and treating children or adolescents.
Additional Requirements:
- Possession of a valid Michigan driver's license and current automobile insurance, along with access to a vehicle during work hours.
- Ability to perform physical tasks associated with the role.
Preferred Qualifications:
- Candidates with a Michigan Bachelor Level Social Worker (LBSW) credential are preferred.
