Nurse Care Coordinator

At The Village Health & Rehabilitation, we are seeking a skilled and compassionate Registered Nurse to join our team. As a key member of our healthcare staff, you will play a vital role in providing high-quality care to our residents.
Job Summary:The Registered Nurse will be responsible for providing direct patient care, administering medications, and collaborating with the interdisciplinary team to develop and implement individualized care plans. This is a challenging and rewarding opportunity for a dedicated and experienced nurse to make a meaningful difference in the lives of our residents.
Key Responsibilities:- Provide direct patient care, including administering medications, wound care, and vital sign monitoring.
- Develop and implement individualized care plans in collaboration with the interdisciplinary team.
- Collaborate with healthcare providers to ensure accurate and timely communication of patient information.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
- Supervise and mentor nursing staff to ensure high-quality care and adherence to facility policies and procedures.
- Current, unencumbered RN license in the applicable state.
- Minimum one year of experience in long-term care or gerontology.
- Strong communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
- Flexibility to work varied shifts, including nights, weekends, and holidays.
