We are seeking a highly skilled Registered Nurse to join our team in the Cardiac/Pedi unit at Yale New Haven Health. As a key member of our healthcare team, you will be responsible for providing exceptional patient care and working collaboratively with our multidisciplinary team to achieve outstanding patient outcomes.
Key Responsibilities- Assess and evaluate patients' physical and psychosocial conditions to develop and implement individualized care plans.
- Collaborate with the healthcare team to coordinate patient care and ensure seamless transitions throughout the continuum of care.
- Provide education and support to patients and families to promote health and wellness.
- Supervise and delegate tasks to non-RN personnel to ensure efficient and effective care delivery.
- Participate in quality improvement initiatives to enhance patient care and safety.
- Graduate of an accredited school of nursing with a BSN preferred.
- Valid State of Connecticut Registered Nurse license.
- BLS certification required.
- Proficient in the use of computer software, including electronic patient records.
