About the Role

As a Case Manager, you will be responsible for:

  • Collaborating with internal and external partners to provide comprehensive care and support to youth
  • Teaching life skills and promoting independence in youth
  • Developing and implementing individualized plans to meet the unique needs of each youth
  • Providing emotional support and guidance to youth and their families

Requirements

To be successful in this role, you will need:

  • A Bachelor's degree in Social Work, Psychology, Sociology, or a related field
  • At least four years of experience in human services or a related field
  • Experience working with at-risk youth and group facilitation
  • CPR certification and a Class E Driver's License
