Job Summary:

We are seeking a skilled RN Case Manager to join our team at SSM Health. As a key member of our care coordination team, you will play a vital role in ensuring patients receive seamless care transitions from hospital to home.

Key Responsibilities:
  • Assess patients' discharge planning needs and develop individualized plans to ensure safe and effective transitions to next level of care.
  • Collaborate with interdisciplinary teams to identify and address barriers to discharge, including social determinants of health.
  • Utilize evidence-based practices and clinical expertise to optimize patient outcomes and reduce readmissions.
  • Communicate effectively with patients, families, and healthcare providers to ensure timely and efficient care transitions.
  • Participate in quality improvement initiatives to enhance care coordination processes and patient satisfaction.
Requirements:
  • Graduate of accredited school of nursing or equivalent education.
  • Three years' acute care experience, preferably in case management or care coordination.
  • Registered Professional Nurse (RN) licensure in Illinois, Missouri, Oklahoma, or Wisconsin.
