Job Description

Schedule:

Monday - Friday, 8:30a to 5:00p, rotating nights, weekends and holidays


We are looking for an Admissions Coordinator to join our team.

The role will include:

  1. Reviewing all referrals submitted by hospitals for admission to rehab and/or LTC
  2. Keeping record of LTC applications and following up with family inquiries regarding placement and billing
  3. Maintaining open communication with hospital discharge planners on acceptance or denial of patient referrals
  4. Ensuring required referral information and data are obtained timely prior to an admission arriving
  5. Entering patients in the Electronic Medical Record (EMR)
  6. Following up daily on all pending referrals
  7. Bridging communication with the team regarding new admissions
  8. Having a basic understanding of insurance, electronic medical records and medical terminology
  9. General use of Microsoft Office Tools

This role will also learn basics of MDS to assist the Care Coordination Group as well as other duties to complete the team goals.

Qualifications:
Current nursing license (LPN/RN). Must have a genuine interest in geriatrics and assuring quality medical records on residents. Ability to adapt to frequent changes in routine. Minimum of one year in medical field and computer experience preferred.

Estimated Salary: $45,000 - $55,000 per annum based on location and experience.
