Leader in Patient Care Coordination

At SouthernCare, we're driven by a passion for delivering exceptional patient care, and we're seeking a seasoned healthcare professional to join our team as a Patient Care Manager - RN.
As a leader in patient care coordination, you'll be responsible for overseeing the day-to-day operations of our hospice sites, ensuring the highest quality of care for our patients and their families. Your clinical expertise and leadership skills will be instrumental in driving our mission to deliver compassionate and dignified care.
Key responsibilities will include managing site operations, administering clinical aspects of the hospice program, and ensuring the provision of quality care to patients. You'll be a key member of our interdisciplinary team, working closely with colleagues to provide patient-centered care.
We're looking for a candidate with a strong background in nursing, preferably with a Bachelor's degree and at least three years of experience in direct patient care. You'll also need to have a minimum of one year of management/supervisory experience and be licensed to practice nursing in the state of employment.
In addition to your clinical expertise, you'll need to possess excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams. You'll also need to be sensitive to the needs of terminally ill patients and their families, with a strong commitment to patient advocacy.
We offer a comprehensive benefits package, including health insurance, 401(k) plan, tuition reimbursement, and paid time off. You'll also have opportunities for professional growth and development, as well as a competitive salary ranging from $80,000 to $110,000 per year, depending on experience.
About You:
- Registered Nurse with a minimum of Associate's Degree in Nursing required
- Bachelor's Degree in Nursing preferred
- Three years' experience in direct patient care in a clinical setting required
- Minimum one year of management/supervisory experience required
- Two or more years' experience in hospice or home care required
- Current license to practice nursing in the state of requested employment
Specialized Knowledge and Skills:
- Knowledge of terminally ill patients and their families along with understanding of hospice concept
- Excellent patient assessment skills
- Good oral and written communication
- Thorough knowledge of managed care principles, regulatory guidelines (i.e., Medicare, Medicaid, and human resource) management principles
