Lead Patient Care Facilitator

Key Responsibilities- Manage the primary functions of the Patient Care Coordinator II.
- Facilitate level-loading during both scheduled and unscheduled absences to maintain continuity of care.
- Provide cross-coverage for designated Patient Care Coordinator roles as needed.
- Engage in the creation and enhancement of training materials to support the development of team members.
