Position Overview

Key Responsibilities
  • Manage the primary functions of the Patient Care Coordinator II.
  • Facilitate level-loading during both scheduled and unscheduled absences to maintain continuity of care.
  • Provide cross-coverage for designated Patient Care Coordinator roles as needed.
  • Engage in the creation and enhancement of training materials to support the development of team members.
