Position Overview
The University of Washington Medical Center - Montlake is seeking a dedicated Patient Care Coordination Nurse to enhance our Social Work and Care Coordination department. This role is pivotal in ensuring a seamless patient experience throughout their hospital stay, leading to a safe and efficient discharge process.
Key Responsibilities
- Identify and monitor patients with significant medical needs to ensure timely completion of necessary consults, procedures, and tests.
- Facilitate ongoing communication among the healthcare team, ensuring alignment on the patient's care plan.
- Lead interdisciplinary huddles to address and eliminate barriers to discharge related to medical issues or systemic challenges.
- Assist in the procurement of complex medical equipment and support safe transitions of care.
- Develop and track outcome measures related to care coordination, focusing on staff and patient satisfaction with discharge planning.
- Provide ongoing education and consultation to staff to improve patient flow through the healthcare system.
Qualifications
- A Master's degree in Nursing or a related field; applicable experience may be considered in lieu of formal education.
- A minimum of three years of professional nursing experience, with a strong preference for candidates with Case Management experience.
- Proficient in English communication, both verbal and written.
- Strong interpersonal skills, leadership capabilities, and clinical expertise.
- Ability to access and document information in electronic medical records.
- Current RN licensure in Washington State and BLS certification required at the time of hire.
