LPN - Licensed Practical Nurse - SF Village - PT Evenings

**Job Summary:**
The LPN provides professional nursing care for residents of all ages in long-term care, under the supervision of a registered nurse, advanced practice provider, or physician.
**Responsibilities:**
* Provides technical support to healthcare professionals as needed
* Administers medications or treatments as ordered
* Documents resident interactions and outcomes related to resident care, medication, behavior, activity, and dietary
* Counsels residents, family members, and/or resident advocate on prevention and treatment plans
* Performs other related duties as assigned
**Requirements:**
* Graduate from an accredited practical nursing program as a Licensed Practical Nurse
* Associate degree in practical nursing is preferred
* Currently holds an unencumbered Licensed Practical Nurse (LPN) license with State Nursing board and/or possess multi-state licensure privileges
