We are seeking a skilled and compassionate Hospice Nurse Practitioner to join our team at BrightSpring Health Services. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality care to patients and their families.
Responsibilities- Provide medical care and palliative services to patients with terminal illnesses, in compliance with state Nurse Practice Act and under the supervision of a collaborating physician.
- Conduct thorough history and physical examinations to assess patients' medical conditions and health histories.
- Complete hospice face-to-face visit requirements to assess eligibility for certification and recertification.
- Make recommendations to the hospice medical director regarding patient eligibility for certification and recertification.
- Maintain accurate clinical notes and communicate effectively with physicians and interdisciplinary team members.
- Ensure compliance with all state and federal legal and regulatory requirements.
- Graduate from an accredited School of Nursing and accredited Nurse Practitioner program.
- Current DEA license, if applicable to state regulations.
- Current nursing license in state of practice.
- Current CPR certification.
- Master's degree with minimum of one year NP experience.
- One year hospice or palliative care experience preferred.
- Accreditation from an approved certifying body for advanced practice nursing as required by state of practice.
- Certification in a specialist area preferred (e.g. Hospice and Palliative Nursing (CHPN) or Geriatrics).
