RN Hospice House- Richard Nau Hospice House

We are seeking a skilled and compassionate RN Hospice House- Richard Nau Hospice House to join our team at PeaceHealth. As a key member of our interdisciplinary team, you will provide direct skilled nursing care and coordination/management of patients admitted to our hospice house inpatient services.
Key Responsibilities- Perform initial evaluations of patients and apply admission criteria to determine if they are safe candidates for home health/hospice services.
- Provide skilled nursing care to patients and skilled education/instruction to patients and caregivers according to the hospice plan of treatment.
- Develop, coordinate, modify, and manage written individualized plans of care, as ordered by the primary physician, to meet patients' long and short-term medical goals and appropriate/timely hospice outcomes.
- Coordinate care, collaborate, and communicate with other members of the interdisciplinary team to ensure appropriate and smooth flow of care/services through the care continuum.
- Perform discharge planning appropriately towards patient-specific goals/outcomes and evaluate interventions and outcomes of patient care by reviewing long and short-term goals established in the plan of care.
- Provide accurate, complete, and timely clinical documentation in compliance with regulatory, quality, and Agency guidelines.
- Assist with the plan and implementation of family bereavement services for patients following death.
- Accredited School of Nursing Required: Nursing
- Bachelor's Degree Preferred: Bachelor of Science in Nursing
- Minimum of 1 year Required: Home health or hospice/end of life nursing experience
- Preferred: Hospice experience and demonstrated knowledge of and ability to apply age-specific principles of growth and development and life stages to meet each patient's needs
- Required Upon Hire: Registered Nurse in state of practice
- Required Upon Hire: Basic Life Support
- Required Upon Hire: Food Handlers Card - Washington
This position requires the ability to lift patient handling no greater than 35 lbs. without the use of assistive equipment and/or devices. Fine motor skills are required to grasp and control medical equipment and perform precise procedures. The ability to move around the area with frequent sitting is also required.
Exposure to biohazard, body fluids, and airborne particles is a potential risk. The ability to complete tasks in a noisy environment is also necessary.
The ability to communicate and exchange accurate information is essential. Vision and hearing within normal limits are required (glasses, contacts, hearing aids permitted).
