Responsibilities
  • Provide physical therapy services to patients in a Hospice setting, including assessment, treatment, and education.
  • Collaborate with interdisciplinary teams to develop and implement patient care plans.
  • Conduct thorough assessments of patients' physical abilities and develop individualized treatment plans to address their needs.
  • Implement and evaluate treatment plans, making adjustments as needed to ensure optimal patient outcomes.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
Requirements
  • At least 1 year of relevant specialty experience in a Hospice setting.
  • Strong clinical skills and ability to work independently.
  • Excellent communication and interpersonal skills.
  • Ability to work in a fast-paced environment and prioritize tasks effectively.
