Job Summary

We are seeking a skilled Registered Nurse to join our team at Eden Health. As a Hospice Registered Nurse, you will provide in-home services to our patients in the Longview, WA area.

Key Responsibilities
  • Provide nursing services requiring substantial and specialized skill, in accordance with the plan of care.
  • Manage a caseload of 12-18 patients, varying in complexity and geography.
  • Develop and implement individualized care plans, addressing physical, emotional, psychosocial, and spiritual needs.
  • Collaborate with interdisciplinary teams to ensure continuity of patient care.
  • Educate patients and families on their rights and responsibilities as Hospice patients.
Requirements
  • Graduate of an accredited nursing program.
  • Current licensed Registered Nurse (RN) in the state of practice.
  • Experience in acute-care, home health, community health, or Hospice settings preferred.
  • Valid driver's license and reliable transportation.
  • Proof of up-to-date COVID vaccine or exemption request.
