Key Responsibilities
  • Assess patients' health status through physical examination and diagnostic testing.
  • Administer medications and treatments as prescribed by physicians.
  • Participate in care planning and development of individualized care plans.
  • Supervise and mentor LPNs and certified nursing assistants to ensure high-quality care.
  • Communicate effectively with physicians and other healthcare professionals to ensure seamless care coordination.
  • Document patient assessments and care in accordance with standards of care and company policy.
  • Educate patients and their families on health-related issues and promote health literacy.
Requirements
  • Graduate of an accredited school of nursing with a valid LPN license in Colorado.
  • Current CPR certification.
  • One year of experience in a long-term care environment (preferred).
