About the Role

We are seeking a highly skilled and compassionate Licensed Practical Nurse to join our team at Sunrise Senior Living. As a Wellness Nurse, you will play a vital role in ensuring the overall well-being of our residents.

Responsibilities
  • Identify and address the ongoing needs and services of residents to promote high-quality resident care.
  • Ensure proper follow-through and participate in evaluations and documentation for residents with changes in healthcare services and status.
  • Assist the Resident Care Director in conducting resident evaluations and gathering data at various intervals.
  • Conduct monthly wellness visits for all residents and monitor and follow up on residents who have experienced falls or incidents.
  • Document all pertinent information in the resident wellness file and maintain effective communication with residents' families and responsible parties.
  • Collaborate with attending physicians and ensure medical needs are met.
  • Ensure weights and vital signs are obtained monthly for each resident and maintain wellness files according to Sunrise policies and federal and state regulations.
  • Provide clinical support and assistance to community team members as needed and maintain infection control practices.
  • Participate in quality assurance meetings and clinical meetings to promote resident-centered care and quality outcomes.
Qualifications
  • Graduate of an approved college or school of nursing.
  • Current state license as a professional Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN).
  • Minimum of one year experience in home health, assisted living, hospital, or long-term care environment.
  • Demonstrated knowledge of nursing practices, techniques, and methods applied to health and wellness resident care, and federal, state, and local regulations.
  • Ability to handle multiple priorities, possess written and verbal skills for effective communication, and demonstrate good judgment, problem-solving, and decision-making skills.
